Lumbar Decompression

Learn how lumbar decompression surgery relieves spinal nerve pressure and restores mobility. Explore minimally invasive options with Dr Mark Tan BH.

Lumbar decompression refers to a group of surgical procedures designed to relieve pressure on spinal nerves in the lower back. These include minimally invasive tubular and endoscopic techniques, as well as open surgery for more complex cases.

What Is Lumbar Decompression?

Lumbar decompression is a surgical approach commonly used to treat spinal stenosis, a condition where the space for the nerves is narrowed by disc bulges, arthritic bony growth, and ligament overgrowth. Symptoms may include pain, numbness, or weakness in the lower limbs.

It is a routine procedure that relieves nerve constriction through the careful removal of the posterior bony aspect of the spine (lamina) and ligamentum flavum.

Indications for Lumbar Decompression

Lumbar decompression surgery is typically indicated when the following conditions are present:

  • Intractable pain despite conservative management
  • Severe circumferential compression on nerves
  • Presence of neurological deficits such as numbness or weakness

These symptoms are often seen in patients diagnosed with lumbar spinal stenosis or nerve compression in the lower back. Lumbar decompression is considered when conservative treatments have not provided relief.

Types of Lumbar Decompression Surgery

There are several types of lumbar decompression surgery, each tailored to the patient’s condition, anatomy, and severity of nerve compression. The techniques range from minimally invasive approaches to traditional open surgery, all aimed at relieving nerve pressure and improving mobility.

Minimally Invasive Tubular Lumbar Decompression

This is a minimally invasive lumbar decompression surgery technique that involves a 2 cm incision on the side of the spine. Sequential dilators are used to split muscle fibers prior to bony resection of the lamina and ligaments causing compression.

This method allows for faster recovery and reduced postoperative discomfort compared to traditional open surgery.

Minimally Invasive Endoscopic Lumbar Decompression

This is a pinhole lumbar decompression surgery approach involving two 1 cm incisions on the side of the spine. Endoscopes or viewing scopes are used along with specialized instruments to perform bony resection of the posterior lamina and ligaments that cause compression.

Endoscopic techniques aim to preserve normal tissue and enhance postoperative mobility.

Open Lumbar Decompression Surgery

Open lumbar decompression is performed via a midline incision over the posterior aspect of the spine. Muscle tissue is dissected to expose the posterior bony aspect of the spine. Removal of bone and ligaments from this area provides space for the nerves, alleviating symptoms.

Open techniques are generally reserved for more severe or complex cases requiring direct visualization and access.

Planning the Right Surgical Approach

The choice of lumbar decompression technique depends on the patient’s condition, severity of nerve compression, and overall health. Minimally invasive methods are preferred for less severe cases due to smaller incisions and faster recovery. Open surgery is reserved for more complex situations requiring direct access.

A thorough assessment by a spine specialist helps determine the best approach to effectively relieve symptoms while reducing risks and aiding recovery.
